Clinic/Health Services Information


Parents and guardians should complete the annual Student Information Update online via www.kleinisd.net  fr each student at the beginning of the school year. Please include any health or wellbeing matters specific to your student. These could include medication to be administered at school, allergies, chronic health conditions, pertinent medical history, or special dietary accommodators.
Health Services documents for your completion can be located on www.kleinisd.net under the Health Services Department.

If any medication medication should be dispensed at school, the parent or guardian should complete and sign the American Authorization Form and bring the medicine to school in its original pharmacy container. Parent or guardian is responsible for transfer of medications from home and other district campuses. School personnel and students cannot transfer or transport medications. Medications are not sent home with students. Student cannot report to school in possession of any medication, prescribed or over the counter. Violation could result in disciplinary action.

Sarah Benson, BSN,RN is the clinic nurse and Kathryn Loomis, EMT is the clinic assistant for the Therapeutic education Program. Please contact the clinic to schedule a time to drop off medication to be dispensed at school, or to provide information related to your student's health.
